Jacqueline Kurth is the Office Chief for Injury and Violence Prevention in the Bureau of Chronic Disease and Health Promotions at the Arizona Department of Health Services. Jacqueline's office is responsible for managing several federal grants and programs aimed to reduce domestic and sexual violence; child maltreatment, suicide, substance use and, opioid disorder and fatal drug overdose. 

Jacqueline has a Bachelor of Science Degree in Speech Pathology and Audiology from Colorado State University; and, a  Master of Arts Degree in Rehabilitation Counseling from the University of Northern Colorado. Jacqueline is a nationally certified Rehabilitation Counselor (CRC); Medical Case Manager (CCM); and a Certified Life Care Planner (CLCP).
