Dr. Ambrosia is an interventional cardiologist who graduated from the College of Osteopathic Medicine of the Pacific after completing his undergraduate degree in engineering at Arizona State University. An Arizona resident since 1976, he completed his residency in Internal Medicine and fellowship in Cardiology at Good Samaritan Regional Medical Center in Phoenix. Beginning practice in Mesa in 1998, he originally practiced at Mesa General Hospital and moved his practice to the Banner Heart Hospital when it opened in 2000. A founding member of his current group, CardioVascular Associates of Mesa, he specializes in interventional and structural heart procedures and was involved in the development of many of the interventional and structural heart procedures at Banner Heart Hospital.
Dr. Steven Boles received his D.O. degree from C.O.M.S. in 1981; interned at Phoenix General Hospital and then received additional residency training in Internal Medicine.
He was trained in Addiction Medicine at St. Luke’s Behavioral Health Center in Phoenix, AZ, from April 1999 through August 2000, by C.J. Shaw, M.D., and the nursing staff.
Dr. Boles was initially certified in Addiction Medicine by the
American Society of Addiction Medicine (ASAM), in 2000, and then recertified in 2008. He became an ASAM Fellow in 4/2008, after 8 years of preparation.
Dr. Boles was in the first group of physicians that were granted Board Certification and Diplomate status by the American Board of Addiction Medicine, in 2009.

He has developed and been privileged to teach an Addiction Medicine Course to 1st & 2nd-year Osteopathic medical students at AZCOM-Midwestern University for the last 26 years.
He has maintained his practice for over 42 years in Phoenix, AZ.,
where he is supported by his beautiful wife Natalie, and their 4 adult children.
He has become a dog lover over all this time and has 3 miniature Australian Shepherds.
Dr. Fariba M. Donovan received her medical degree at The Shiraz University of Medical Sciences in Iran and completed her internal Medicine residency at Good Samaritan Hospital in Cincinnati, Ohio. She then completed her infectious disease fellowship at the University of South Florida in Tampa, Florida.
During her graduate studies in Japan after her medical school her research focused on C. albicans virulence factors. She later in postdoctoral fellowship focused on Coccidioides urease as a virulence factor and was able to mutate the gene in Coccidioides and constructed a less virulent strain.
She joined the faculty at the University of Arizona in 2017 and has been focusing on early pathogen-host interaction in coccidioidomycosis. She is studying early coccidioidomycosis events to better understand altered or dysfunctional metabolic pathways as evidence of genetic alterations and a risk factor for severe coccidioidomycosis or disseminated coccidioidomycosis (DCM).
Her clinical research focuses on the earlier and improved diagnosis of Valley fever. Her clinical research goals include the development of more rapid, reliable, and cost-effective Valley fever point-of-care testing which will speed up and improve diagnostic accuracy and antimicrobial stewardship. She completed a prospective study evaluating a rapid LFA test for Valley fever diagnosis. Her earlier work has demonstrated the human and financial burden of delays in Valley fever diagnosis. Her work epitomizes the goal of taking basic research and applying it at the bedside for the benefit of the entire community. Additionally, she is developing plans to study the host innate immune response to Coccidioides with a focus on the early events in coccidioidomycosis.
Dr. Horlick is a Board-Certified Pediatrician with over 25 years of experience in the Phoenix area. After relocating to Arizona from central Illinois, Dr. Horlick attended the University of Arizona College of Medicine and completed her pediatric residency at the Medical College of Wisconsin, Children’s Hospital of Wisconsin. Since returning to Arizona in 1999, she has been dedicated to providing high-quality pediatric care and has recently joined Midwestern University as an Assistant Professor of Pediatrics in January 2025.
A Fellow of the American Academy of Pediatrics, Dr. Horlick is deeply passionate about preventive medicine and its ability to empower families to live healthier lives. She has a particular interest in nutrition, fitness and eating disorders, areas she believes are vital in combating the current obesity epidemic. To expand her expertise, she joined the Phoenix Children’s Hospital CARE team, where she honed her skills in caring for children and adolescents affected by obesity and its related conditions. In 2022, Dr. Horlick furthered her commitment to this field by attaining Board Certification in Obesity Medicine, and she aims to contribute to the growth of a Pediatric Obesity Program at Midwestern University.
Outside of her professional endeavors, Dr. Horlick enjoys an active lifestyle, including yoga, Peloton cycling, and healthy cooking. She values spending time with her family and lives in the Phoenix area with her husband, Mitch, and their two children, who are currently in college.
Wendy Huang, MD is a board-certified Ophthalmologist with subspecialty training in adult and pediatric eye diseases. She graduated in 2003 from the University of Chicago with a degree in Biology, where she was awarded a Howard Hughes fellowship in research. She attended medical school at Rush University in Chicago in 2008 and went on to complete ophthalmology residency at The New York Eye and Ear Infirmary in 2012. She specialized in Pediatric Ophthalmology and Adult strabismus at the Children’s Hospital of Los Angeles and the Doheny Eye Institute/USC in 2012. After a decade in practice, she obtained further expertise and completed a fellowship in Uveitis and Cornea and External Disease at The University of California, Irvine-Gavin Herbert Eye Institute in 2023. Her focus is on ocular inflammatory disorders, cornea, and complex pediatric eye diseases, including pediatric uveitis.

Dr. Huang is an Associate Professor at Midwestern University and is on faculty as a Clinical Assistant Professor of Ophthalmology at the University of Arizona College of Medicine-Phoenix. She has memberships in the following national organizations: American Board of Ophthalmology, American Academy of Ophthalmology, American Academy of Pediatrics, American Academy of Pediatric Ophthalmology and Strabismus, Cornea Society, and American Society of Cornea and Refractive Surgery.
Dr. Keane currently works as an Assistant Professor, Medical Director of the Osteopathic Medicine Center Arizona and Chair of the Osteopathic Principles and Practice Department within the School of Osteopathic Medicine in Arizona on the A.T. Still University Mesa Campus. In his continued learning, he has been focused on pain management, especially chronic pain and the associated hypo-dopaminergic state, as well as treatment of opioid addiction, PTSD, and neurophysiologic mechanisms of pain. Having recently moved from the clinical to medical education arena, he is excited to serve as co-investigator for the proposed project, Noninvasive multiparametric characterization of myofascial pain syndrome and its responses to osteopathic manipulative treatment.
After decades of managing on his own health and wellness interests, initially with Tai Chi/Traditional Chinese Medicine and self-guided learning about nutrition, he left a developing career in public policy to more directly use the knowledge and skills he had learned to help others with their health. The idea of becoming a physician to help reduce suffering was irresistible. After several years of pre-medical training, he was accepted to medical school. After completing an internal medicine residency and board certification (he is currently a Fellow in the American College of Osteopathic Internists and in the American College of Physicians), a secondary board-certification in integrative holistic medicine and extensive post-graduate training in osteopathic manipulative medicine, he continues to look for opportunities to enrich his understanding and practice of medicine. Thus, he is especially grateful for this opportunity to blend the art of osteopathic clinical care with the science of evidence based assessment and objective analysis. He feels that his experience, particularly with regard to clinical care, osteopathic principles, and osteopathic manipulative medicine, makes him qualified to provide expertise in the design, development, and implementation of the study proposed in this application.
Dr. Joanna Kramer earned her BS in Biology at Ursinus College, her MPH from University of Nevada Las Vegas, and her DO degree from Touro University Nevada. She then completed an AOA/ACGME dually accredited pediatric residency and a chief residency at Nationwide Children’s Hospital in Columbus, Ohio. She is a faculty physician in the Phoenix Children’s Hospital Division of Primary, Complex Care, and Adolescent Medicine. She is an associate professor of clinical medicine at the University of Arizona College of Medicine Phoenix and a clinical instructor at Mayo Alix School of Medicine Phoenix. She is core faculty for the Phoenix Children’s Pediatric Residency Program Alliance, rotation director and track director for osteopathic education, and creator of the Pediatric Osteopathic Didactic Series. She is an executive committee member of the American Academy of Pediatrics Section on Osteopathic Pediatricians. She is passionate about resident education, osteopathic research, public health research, and quality improvement.
Dr. Levitt is an adult and forensic psychiatrist. She has been working in Arizona for almost 35 years. She is an attending psychiatrist at Valleywise Behavioral Health and has a private practice offering forensic consultations.
Dr. Levitt graduated from residency at Maricopa Medical Center (now Valleywise.) Her practice as an adult psychiatrist has always focused on the Seriously Mentally Ill. She has worked in various hospitals both public and private. She worked in the correctional field for over 10 years. Her forensic practice currently focusses on fitness for duty and guardianship work, but she also continues to provide consultation in the civil and criminal arena.
Dr. Lopa Misra is an Associate Professor of Anesthesiology and Perioperative Medicine at Mayo Clinic in Phoenix, Arizona. Dr. Misra is also the Associate Dean of Admissions for Mayo Clinic Alyx School of Medicine Arizona Campus. Her interests include medical education, health equity, humanities in medicine, and regional anesthesia and acute pain. Dr. Misra is a certified life coach and is a sought-after speaker nationally and internationally. She is wife and a mother to two children, 19 and 15 years of age. Her personal interests include reading, traveling, dancing, and hiking.
Sristi Nath, DO is a board-certified general and child and adolescent psychiatrist who has been in private practice in Scottsdale, Arizona, since 2007. In addition to medication management, she also provides psychotherapy and promotes mind-body treatment strategies. She received a BA in Psychology at Stanford University and a D.O. Degree from the Arizona College of Osteopathic Medicine (AZCOM). She completed her residency in general psychiatry and fellowship in child and adolescent psychiatry through the Hawaii Residency Programs. In addition to her private practice, she has taught at AZCOM as a regular lecturing professor since 2009.
Dr. Nath’s philosophy is strongly rooted in observing the whole person and using a BioPsychoSocial formulation to inform treatment choices. Osteopathic principles, mind-body awareness, and cultural humility are incorporated into all patient care. She values her free time with her family as well as teaching and practicing vinyasa yoga.
Dr. Ambrose F. Panico, is a board certified Clinical Cardiac Electrophysiologist and is the Medical Director of EP at Cardiovascular Associates of Mesa. In addition, he is the Medical Director of the Cath/EP Lab at Banner Heart and is an Assistant Clinical Professor of Medicine at the University of Arizona - Phoenix.
Dr. Panico shares his philosophy on being a medical professional. “It is an honor and a privilege to practice medicine and the physician-patient relationship is what makes my job so special. Each patient is unique and deserves an individualized treatment plan, which we will work together to create. I will ensure that patients feel comfortable, confident and empowered to improve their cardiovascular and overall health.”
Dr. Raney received her Doctor of Pharmacy degree from the University of Arizona and
then completed postgraduate residency training in primary care at the Medical College of Virginia Hospitals in Richmond, Virginia. She is currently a Professor of Pharmacy Practice at Midwestern University College of Pharmacy in Glendale, Arizona. She practiced as a clinical pharmacist in the Midwestern University Family Medicine Clinic, providing collaborative disease management services for patients with chronic cardiometabolic conditions for 15 years. Her primary teaching responsibilities and clinical interests are in the areas of endocrinology, with an emphasis on diabetes and women’s health. Her scholarship efforts focus on the development and evaluation of continuous professional development strategies for students and practicing clinician educators.

Valerie Ruiz is the Program Manager of the Medical Resource Office (MRO) Division at the Industrial Commission of Arizona (ICA). With more than two decades of experience in healthcare, she brings a wealth of clinical expertise and program management acumen to her role. In the upper right corner, click on sign in or, if it has your name, click on your name.
On your main account page, click the My List Tab, find the AOMA 46th Annual Fall Seminar.
Click the gold certificate button.
Then Click on Affidavit
Read the disclosures and click I agree.
Click on Customer Attendance Confirmation.
You must attest to each lecture you viewed to receive credit. Make sure you scroll to the bottom and click on submit before closing.
If you do not check all the lectures viewed at one time, when you return to check the remaining lectures, you will need to click on the edit button.
Each lecture you clicked on will then appear below. You must click on each individual lecture and click on attended or did not attend. There are additional questions you can answer. They are not mandatory, but we appreciate the feedback. Be sure to scroll to the bottom and click Submit.

There are four Opioid Credits available. We will email you a separate Opioid CME certificate in January 2027. If you need it before that, please let me know.
